Push-creates-patch: auto-create patch on refs/for/* push

When a user pushes to refs/for/<branch> on the server, the SSH receive handler should automatically create a new patch (or update an existing one for that branch) without requiring a separate 'collab patch create' step. This is the core Gerrit UX that makes the push-to-review workflow feel natural.
